Transitional lumbar anatomy

I have transitional lumbar anatomy and I'm not sure where to go from here. I was diagnosed with hip dysplasia at birth. I've seen studies that explore the possibility of hip dysplasia and transitional lumbar anatomy. I guess the diagnosis of a spinal anomaly would have been helpful all these years. I've been in pain since I was 15. So it's sometimes diagnosed as sacral pain, sciatica and anterior femoralas parasthetia. I've heard all of them. Personally I would like to have surgery. I've done injections and pt. How do I let the doctor know I want surgery? I'm in so much pain. I don't know how much more pain, Dr appts, co-payment or deductibles I can take.
