25 years old, Lumba Laminectomy 6 years ago, now possible fusion

Long story short because I’m sure no one wants to read an essay. At the age of 19-20 i had to get a L5 S1 laminectomy. Fast forward to now me being 25 years old. My pain is so unbearable that i cry at night until it goes away. Couldn’t afford health insurance so that’s why iv lived with this for so many years. Now i have health insurance and my doctor says i need an Mri. My possible options is either an epidural or surgery because he says i might have a ruptured disc now. I got the laminectomy prior because of a bulging disc. So I’m assuming ima need spinal fusion surgery. So i need advice. Should i go with the epidural or just take my chances with surgery? I can’t keep living with this pain anymore. Thank you for your time!
