Are you in the us? If yes you can fill out the form online. I began the process 10 mos. ago. I received a denial, sent an appeal that was denied and have just yesterday sent a form to request a hearing with a judge. I also contacted a lawyer who has given me a form for my surgeon to complete... Once I return that to the lawyer ill wait more and see if anything happens. I am going to tell you the process is long, takes patience and you must keep it going because most everyone is denied a few times. Find a SSD lawyer to help guide you and look over your details. No $$ is given to them unless you get approved and then it is deducted from settlement. Typically they won't even see you until after you file your initial application and receive a denial. Stick with it and don't get discouraged. Be honest and provide with your initial application as much medical record as you can, along with MRI , ct scan reports etc. take the time initially to give them as much info as you can. Just look at it as a job! Because it is! Be sure to make a copy for yourself of everything you send them and keep a journal with dates. I have even gotten a few phone calls so be on the lookout for unfamiliar phone numbers.... Good luck and as you know our status changes all the time.

Hi Janet,,Trish is 100% right,,,I too applied. Do youself a favor and use the lawyere as Trish said,,here in NY their capped fee is 1,000.00$ and they only get paid if U win and it come out of ur retro check..In NY if ur accepted U get 1 yrs back pay of ur sallery and they pay all your outstanding medical bills. If U win Ur medicare kicks in 12 months after acceptance,,BUT, the clock start on that starting from the first day U visited A doc about ur condition,,It could B diff. where U live though..A lawyer is best,,Its not worth all the paperwork and running around,,thats what they are 4. If U dont win U just have to pay the fee they were charged for your medical record copies. All U have to do is show up to the appointment w/ ur diagnosis and list of meds, and all the phone #s and addresses of all physicians U have seen, they obtain the rest, any medical documentation up until the last moment the lawyer will obtain,at the end is when they will ask for all your outstand medical bills,,so dont pay anything!!! Also(N again it could B diff. in ur state) If a medical bill is not paid, meaning hosp. only bills, after one year,,the hospitals get paid from state for reinbursment,,only labs and private physisions go to collections, not the hospital, so save ur dime N dont pay those hospital co-pays or bills. I keep a huge bag N when I receive a bill I dont even open it, I just put it in the bag..MY bag is HUGE,,oh well,,we can only pay out so much. Good luck and yes it is a long process. Rene:)

Thank you for the responses. I looked up the fee for lawyers and the fee in Kansas is 25% not to exceed $6,000.00. (Of course they still get reimbursed for postage, travel, paperwork etc.)
Another ?, Are Dr.s required to give me copies of my med. records if I request them and if so, is there a standard charge for copying these and their time for doing so?
Thanks again for your help,
Janet.

I too have applied, I am only 40. Like Trish, I applied first online and when I received my denial, I got an attorney. I was also told by a friend to contact my congressman. I did and they have also helped. I am awaiting the answers from some type of special hearing, and hoping it soon comes through.

Good Luck, and isn't it terrible, you work all your life, you get hurt of permanently physically unable to work and you have to fight another fight, I don't know about y'all but I honestly don't know how much fight I have left in me.

Hi janet,,about ur medical records..the lawyers will deduct that fee if u win and if u dont win u will have to pay that fee, so yes u R entitled to them for no extra fee, you will B paying for them at the end, U shouldnt have an additional charge from the lawyers to make copies,,the fee is for the cost that the lawyers had to pay for retreiving them. Im still waiting Im about half way there till the decision, I already completed the ssdi doc visit,, thats about the half way there mark. Rene :)

Thanks for all the help. I get all panicky just thinking about yet another fight.

I over did it today, did all the laundry...My daughters say, "Mom, don't do it if it hurts." but with no offer to help...kids! Still, I wouldn't know what to do without them. (Other than a whole lot less laundry).
Enjoy your weekend,
Janet

U got that right Janet,,mine do there own, but god forbid they do mine. Dont U just love it when u have stuff in the dryer ,wet or dry, and they dump it on the floor to put theirs in the dryer.Could U imagine if we pulled that when we were young, my mother would have taken out the wooden spoon, or better yet she was known to throw our stuff out in the snow if we didnt hang up our coats,,U got to luv this new generation, they have no clue,I keep telling them when R u gonna get married n grow up,,they usualy reply ,,Hell NO I aint paying bills. I still have to pay there car payments n insurance, n there nurses making good $,,and every week they come home w/bags from shopping,,If I ask them to p/u milk n hand them a 20.00 I seem to not get any change back,,I know I havnt food shopped in a long time,,has milk gone up that much? haha..Plus they still go to Daddy for gas $$,or Dad I need my oil changed,or new tires and so on..Spoiled brats,,But I luv them,,their my world. The other night when the discussion of getting married came up, my Nicole actually said to her boyfriend of 6 yrs now, I wont marry U until U can maintain my lifestyle that my Dad does for them..Both are Daddys Princesses. They dont even know how to use a light switch..Mario n I laugh N tell them when they have there own home we R gonna come over N turn on every light and leave fingerprints all over,N also eat all there food.About the subject of sleep, 2morro everyone just blog how many hours u got.
