back pain

this is my report from the hospital ...
this patient 28 years old male patinet with no know medical co-morbidities who presented to our clinic with complaints of LBP with right sciatica and right sided thigh pain as well as numbness of the foot for 7 months . his MRI dated 22\5\2011 showed T12/L1 and L3/L4 , L4/L5 , L5/S1 discs with congenital spinal stenosis . he had a previous Ozone therapy procedure with no lasting relief . he was started on pain medication for symptomatic relief failing which an epidural injection was offered . he subsequently underwent a L2/L3 lumbar epidural injection under fluoroscopic guidance on 11/6/2011
