Chronic back pain

I am looking for any advice. Im at the end of my ropes and dont know where else to go anymore. I am post op 13 years now. I had a spinal fusion at L5-S1. I have a grade 2 spondyloisthesis. After surgery, I had no pain for several years. It has now returned worse than ever. I have had xrays and the doctors say that the fusion looks fine. I went for a ct scan yesterday to look at the strenght of the fusion, but dont get the results til friday. My primary doctor has me on metaxalone 800mg. diclofenac sodium 75mg, and vicodin 5/500mg. Have no relief. I have done the physical therapy, that doesnt work either. I am constantly fatigued. I was diagnosed is October of last year with sciatica. I have pain in my lower back, hips, and legs. every now and then numbness in my left thigh. On a pain scale of 0-10, most days I am a 9. I cant stand for more than 20 minutes. I have been so desperate for pain relief, I have tried all kinds of meds from friends and family. I also tried to double or triple my doses of my meds and that does nothing. My orthopedic surgeon says if the ct scan comes back ok, they want to refer me to get steriod injections. I havent heard very good things about them. Im afraid if I say no to the injections, they wont try anyting else. All I want is to be able to normal everyday activities. The way things are going now, I dont think that day will ever come. Any suggestions, would be great.

you should go and have a *neurosurgeon in the area take a look at you / get his 2nd. opinion .
ask questions , ask "his" *success rate of your type surgery , if he recommends it ?

Seems like they would have done a MRI, you get the results a lot quicker and can get a copy of the scan on disc right then, at least a Nuerosurgeon would have something to look at while waiting on the MRI to be read and a report made. A good Nuerosurgeon could probably tell from a Xray almost. I agree with the 2nd opinion with a Neurosurgeon. The Dr. you have doesn't seem to good or concerned to me.

I had a TLIF L5-S1 fusion in 2005 and still have severe pain as well. Maybe you could explore "With a good neurosurgeon" if it could be scar tissue causing the pain. Overtime scar tissue may build in that area as well. Some surgical interventions for this would include Laminectomy or Laminotomy. I am trying to figure out this as well. My Fusion did not take....Pseudarthrosis being the technical term. Rods and Pins still in place but bone did not grow in. I know you have been down the gambit of Physical Therapy but it is extremely important to keep those core muscles strong. Even though I know it hurts like hell to do them when you are in pain. Wow, it seems that all of you who responded to my post is recommending that I see a Neurosurgeon. I will definately explore that option. I dont know why the doctor Im seeing now hasnt asked for an MRI. When I got the CT scan done, that doctor asked my why I wasnt getting an MRI instead. I can only get what my doctor asks for, unfortunately. As far as the physical therapy, I still do all the excersises I was taught. Try to do them at least 3 times a day. I guess I just have to wait and see what happens at my appointment tomorrow. I find out the results of the CT scan. I wish I knew what to look for on that, cause I do have the disc of the scans. Well, went to the doctors yesterday and found out the results of my ct scan. They said that the bone graft fusion I had done 13 years ago never fused. Since it never fused, there is still movement in my spine thats causing my pain. I was furious! Next step was to sit down and discuss my options. First they want me to consult with another doctor in the office and get his opinion. The doctors dont feel comfortable, and not sure if they want to take my case. He mentioned he wanted to send me to Balitmore to see a spine specialist. There is, of course, an option to have surgery again. This time they said to put "hardware" in my back. Thats scary. Im thinking that maybe I should just try and go to pain management and see what they can do for me. I had a bad exoerience the last time I had surgery. I almost died. Had to get flown to Washington DC, in a coma for a week.
So pretty much frustrated and tired. I meet with the other doctor on Wednesday. Guess it wont hurt to get his opinion.

You need Neurontin, (gabapentin) as part of your regime. Mention this to your doctor.

having tried everything last ten years thru the same type surgeries / twice , you've had for chronic pain , for some brief relief have you tried walking in the pool at a athletic club nearby ? besides the limited pain meds / *which seem to dull the level of constant pain , it gets me some relief . i go to walk back & forth for 20 min. / 3 times a week . suprisingly it does help . use the "heated" hot tub spa for 15 min. - *as well . Pain management doctors often are Anathesiologists - *suggestions include physical therapy numerous times , injections , tens electrical pulse unit , prolotherapy , acupuncture visits : * work so little for very long or not at all . surgical implanted morphine *pump / do read up on success / failure rate , if it's brought up . i did , and declined / not for everyone . surgeons seldom bring up chronic pain issues you live with post-op , usually because of the scar tissue area so involved . i had L4-5 -S-1 fusion / hardware / cage wrap done *1st. time / *hardware removed 2nd.
time . surgeries made no difference . pain meds , and "learn to live with it" ... is pretty much the bottomline answer you will hear . i wish you the best . *in the same boat .
